You can also download the form, fill it out, and return it to the office. Please provide a voided check or deposit slip with the completed form.

You may also fill out this form below to sign up for automatic bank draft.

After you fill out the form, your account will be setup to automatically draft out of your bank account. Please provide all necessary information.

Tombigbee Fiber Account Number:
(Ex: 5xxxxx-1xxxxx). Location is the number that starts with "5" or "6"
Customer is the number that usually starts with "1"
